摘要

雌激素與孕激素可促進乳房發育,也會引起乳腺癌風險增長。無論女性還是男性,乳腺癌風險均隨年齡呈指數級增長,而女性風險的增幅遠超男性。在多種情形下,雌激素和孕激素均與乳腺癌風險有關;例如卵巢活動、更年期激素療法、抗雌激素療法、高劑量雌激素療法,等等。
迄今有關雌激素、孕激素對女性傾向跨性別者的乳腺癌風險之影響的研究屈指可數。儘管現有資料有限、且研究方法有侷限性,但有明確證據表明雌激素與孕激素療法可引起女性傾向跨性別者的乳腺癌風險大幅增長,風險率介乎順性別婦女和順性別男性。尚需更多研究來更好地確立女性傾向跨性別者的激素治療與乳腺癌風險之間的關係;尤其是跟蹤時間更長、激素處方不同(例如對比雌激素單方和雌、孕激素複方)的研究。
影響女性傾向跨性別者乳腺癌風險的可能因素有:激素治療時長,開始激素治療的年齡,是否長期並用孕激素,雌、孕激素劑量,等等。相比於有兩條 X 染色體的順性別婦女,順性別男性及女性傾向跨性別者僅有一條 X 染色體,這可能有助於預防乳腺癌。
對於女性傾向跨性別者,儘管乳腺癌的風險會增長,但其終生髮生率並不會高;其風險需持續多年的激素暴露以累積;其通常發生於老年;而且治療乳腺癌也相當容易。因此,對乳腺癌風險的擔憂不足以妨礙到女性傾向跨性別者的激素治療。無論如何,應建議女性傾向跨性別者像順性別婦女那樣,接受乳腺癌篩查。

前言

雌激素與孕激素可促進人類乳房發育。其中,雌激素在青春期乳房發育當中居主導作用;而孕激素在懷孕期間為準備泌乳和母乳餵養而產生的乳房變化當中居主導作用。較之於順性別男性,乳腺癌在順性別婦女當中尤其更為常見;激素被廣泛認為是引起順性別婦女的乳腺癌並發展的因素所在。據此可以合理推斷:女性傾向跨性別者的激素療法或可增加乳腺癌發病風險。本文就此觀點以及既有相關研究項目進行論述。

乳腺癌風險相關因素:性別與年齡

對婦女而言,其患乳腺癌的終生概率通常約為 12.5%(即每 8 人有一人罹患)(Ban & Godellas, 2014);而男性患病概率通常約為 0.1%(即每 1000 人有一人罹患)(Abdelwahab Yousef, 2017)。男性乳腺癌的綜合年化發病率約為每 100,000 人/年一例(Abdelwahab Yousef, 2017; Ottini & Capalbo, 2017)。在所有乳腺癌病例當中,男性佔比不足 1%(Abdelwahab Yousef, 2017; Sun et al., 2017)。綜上,乳腺癌在婦女當中較常見,但在男性當中極為罕見。

除性別之外,年齡是乳腺癌的已知最大風險因素所在(Momenimovahed & Salehiniya, 2018)。對於絕經前婦女,乳腺癌風險隨年齡增長呈指數級上升之勢(Dix & Cohen, 1982)。女性乳腺癌風險在 15 歲時約為 1/600,000;在 20 歲時約為 1/75,000;在 30 歲時約為 1/1,500;到了 40 歲則約為 1/175(Anders et al., 2009)。在其絕經後(通常在 50 歲),乳腺癌風險仍繼續增長,但增幅有所放緩(Dix & Cohen, 1982; Benz, 2008; Colditz & Rosner, 2000)。其原因被認為在於,雌二醇與孕酮水平在絕經後大幅減少(Dix & Cohen, 1982)。據估算,如果不曾絕經,女性乳腺癌病例會多出六倍,也即大部分女性終其一生皆會罹患乳腺癌(Brisken, Hess, & Jeitziner, 2015)。男性的情況與女性類似,其乳腺癌發病率在大約 65 歲之前呈指數級上升之勢,此後變為緩慢上升(Ottini & Capalbo, 2017; Cancer Research UK)。這可能和男性更年期及其導致的更低的睾酮與雌二醇水平有關(Satram, 2006)。女性的乳腺癌確診年齡通常在 60 歲左右,而男性通常在 65-70 歲(Ferzoco & Ruddy, 2016; Giordano, 2018)。下圖展示了女性及男性在不同年齡的乳腺癌發病風險(Ottini & Capalbo, 2017)

女性的激素暴露量與乳腺癌風險的關係

除性別與年齡之外,有研究還強調了卵巢活動及其分泌的激素(如雌二醇與孕酮)是引起乳腺癌的主要風險因素(Ban & Godellas, 2014)。更晚的初潮、更早的更年期、以及母乳餵養,皆可讓乳腺癌風險變得更低,因為上述現象可導致經卵巢分泌的激素總和暴露量更少(Ban & Godellas, 2014; Momenimovahed & Salehiniya, 2018)。如果在 40 歲以前摘除卵巢,可將乳腺癌的終生髮病風險降低 50%(Ban & Godellas, 2014)
對於高患癌風險的已絕經婦女,可長期預防性服用選擇性雌激素受體調節劑(SERM,如他莫昔芬)、芳香化酶抑制劑(AI,如阿那曲唑)等抗雌激素製劑,以預防乳腺癌;其可降低 50-70% 發病風險(Mocellin, Goodwin, & Pasquali, 2019; Nelson et al., 2019)
在靈長類動物當中,長期使用雌激素與孕激素療法被發現可引起乳腺癌,而高劑量則讓發病年齡較常規更早(Cline, 2007)

除增加乳腺癌發病風險之外,卵巢分泌的性激素還會促進活躍的乳腺癌之發展。抗雌激素製劑可有效治療活躍當中的乳腺癌(Lumachi, Santeufemia, & Basso, 2015);而抗孕激素製劑(如奧那司酮米非司酮等)亦可在乳腺癌的治療當中取得初步成效(Palmieri et al., 2014)。如患有乳腺癌的婦女服用生理劑量的雌激素,後者可加速腫瘤的發展(Herrmann, Adair, & Woodard, 1947; Nathanson, 1947; Gellhorn & Jones, 1949; Nathanson, 1951; Pearson et al., 1954; Pearson & Lipsett, 1956; Kennedy, 1956; Kennedy, 1957; Dworin, 1961; Kennedy, 1962; Green, Sethi, & Lindner, 1964; Kennedy, 1964; Stoll, 1973; Forrest, 1989)

和公認的卵巢活動與乳腺癌風險之間關係相關聯的是,更年期激素療法亦可增加更年期、已絕經婦女的乳腺癌發病風險(乳腺癌激素相關因素合作研究小組, CGHFBC, 2019; 圖表)。單服雌激素、或者合用雌激素與孕激素,皆會增加該風險,而後者增加的幅度更大(CGHFBC, 2019)。依據近期一項針對所有已有數據的大型薈萃分析,在更年期激素療法引起的乳腺癌絕對風險上,單服雌激素達 20 年後,發病率將提高約 1.5 倍,而合用雌、孕可提高約 2.5 倍(CGHFBC, 2019)。因此,合用雌、孕療法的乳腺癌發病風險高於單服雌激素療法。在此之前,孕激素因其在乳房中對雌激素的拮抗效應,被認為可能對乳腺癌起抑制作用(Mauvais-Jarvis, Kuttenn, & Gompel, 1986; Wren & Eden, 1996; Gompel et al., 2002);但上述薈萃分析推翻了該假設。
美國“婦女健康倡議”(WHI)項目似乎也和這種由更年期激素療法引起的高乳腺癌風險有關係,至少在一項已使用的處方上(即 0.625mg/天的結合雌激素合併 2.5 mg/天的醋酸甲羥孕酮方案);使用該療法 5 年後,發病風險增加 1.25 倍(WHI 調查員編輯組, WGWHII, 2002; Chlebowski, Aragaki, & Anderson, 2015)。WHI 是迄今規模最大的激素治療隨機對照試驗項目,其為評估關於激素藥物所引起的長期健康效應之因果關係提供了寶貴機遇。

為了將更年期激素療法引起的乳腺癌風險可視化,下圖展示了乳腺癌風險和時間的關係;其基於“護士健康研究”觀察性項目(NHS)的數據與模型(Colditz & Rozner, 2000)。如圖所示,更年期激素療法可導致乳腺癌風險繼續維持指數級增長,而該趨勢本應在更年期開始之後減緩。

同結合雌激素、醋酸甲羥孕酮等非生物同質性激素藥物類似,諸如雌二醇、孕酮等具有生物同質性的激素,也和乳腺癌風險的增長有關(CGHFBC, 2019; 圖表)。服用雌二醇所造成的乳腺癌風險,和服用結合雌激素的風險並無二致(CGHFBC, 2019)。基於現有數據,口服孕酮(5 年以下)與乳腺癌風險的增長並無關聯,這和合成孕酮製劑的情況截然不同(Aly, 2018; CGHFBC, 2019; Stute, Wildt, & Neulen, 2018; Mirkin, 2018; Fournier et al., 2007)。然而,一旦服用超過 5 年,口服孕酮將會助推乳腺癌風險的增長,其增長趨勢在幅度上和服用醋酸甲羥孕酮的情況相似(Aly, 2018; CGHFBC, 2019; Stute, Wildt, & Neulen, 2018; Mirkin, 2018; Fournier et al., 2007)。口服孕酮初期相對較低的發病風險,可能與其產生的孕酮水平較低、從而使孕激素效力更低有關(Aly, 2018; Kuhl & Schneider, 2013; Davey, 2018; 維基百科; 圖表)。因此,在口服孕酮引起的乳腺癌風險之增長足以被準確測定之前,可能需要更長的治療時間(和/或更大的樣本規模)(Aly, 2018; Kuhl & Schneider, 2013; Davey, 2018)
當下對於非口服途徑給藥的孕酮對乳腺癌風險的影響,尚無任何現有數據提供(Stute, Wildt, & Neulen, 2018)。不過,一些研究者預計,無論是長期服用還是短期暴露,非口服途徑、有完全效力的孕酮所引起的乳腺癌風險應和合成孕酮製劑相似(Aly, 2018; Kuhl & Schneider, 2013; Davey, 2018)

儘管無論是前瞻性觀察研究、還是隨機對照試驗所得到的證據皆表明,更年期婦女單服雌二醇的療法可增加乳腺癌風險(CGHFBC, 2019);但是已進行的隨機對照試驗(即 WHI)並未出現單服雌二醇會增加乳腺癌風險的情況,實際上風險反而大幅降低 了——相對風險率(RR)0.77,95% 可信區間 0.64–0.93(CGHFBC, 2019; 圖表)。為了解釋這種矛盾的現象,一些設想被提了出來。例如,在 WHI 項目的隨機對照試驗當中貢獻了絕大部分數據的女性們有些不尋常之處,表現在其開始激素療法的年齡較高(平均 64 歲)、超重和肥胖程度較大等(CGHFBC, 2019; Kuhl, 2005)。超重或肥胖的女性已處於更高的乳腺癌風險當中,可能不受單雌激素療法的進一步影響(Kuhl, 2005)。此外,失去雌激素來源已有多年的婦女在接受治療後可能會對其乳腺癌風險產生反常(paradoxical)的抑制效應,這和從更年期起接受治療的婦女的情況截然相反;從觀測數據亦可見之(Prentice, 2008; CGHFBC, 2019)。這種設想被稱為“空檔假設(gap hypthesis)”(Palmieri et al., 2014; Mueck & Ruan, 2011)
為驗證這些設想,並查明雌激素對更年期乳腺癌風險的影響,尚需更多隨機對照試驗支持。

同樣反常的是,在一些其它特定情形下,雌激素和/或孕激素的暴露亦可降低乳腺癌風險、抑制乳腺癌發展。例如,年輕婦女(35 歲以下)如懷孕超過 34 周,在短期(十年內)有更高的乳腺癌風險,但長期(20–25 年以上)風險反而變低了;其終生風險可降低至多 50%(Nichols et al., 2019; Husby et al., 2018)。懷孕期間,雌激素與孕酮可達到極高水平(例如,34 周後雌二醇可達 12,500 pg/mL 以上)(圖表),而這些激素在動物實驗當中被認為可起到抑制作用(Rajkumar et al., 2003; Hilakivi-Clarke et al., 2012)。對於已絕經 5 年以上的婦女,高劑量雌激素療法用於治療乳腺癌很有效,與抗雌激素療法基本相當(Bennink et al., 2017)。與此相反的是,高劑量雌激素療法對於絕經前婦女的乳腺癌並無作用,儘管其似乎在超大劑量下會起效(Bennink et al., 2017)
然而,雌、孕激素的這些益處只在反常情形下發生(例如超高激素水平/攝入、和/或在性激素不足之前推延之),與女性傾向跨性別者的關係並不密切。

男性的雌激素暴露與乳腺癌風險

對於順性別男性,睾酮暴露量較低、和/或雌激素暴露量較高的情形可提高其乳腺癌風險(Ferzoco & Ruddy, 2016)。這些情形包括:睾丸疾病、睾丸被摘除、肝臟疾病等等(Abdelwahab Yousef, 2017)。其中,睾丸功能障礙、睾丸缺失可導致睾酮暴露量減少;而肝臟疾病會損害雌激素代謝功能,引起雌激素水平上升。此二種情形使得雌激素/雄激素比例增大,從而引起雌激素對乳房更大的刺激作用。乳腺癌風險的增長可能與其存在關聯。同樣地,男性發生的良性乳房疾病、男性女乳症等等也被發現和乳腺癌風險有關(Sasco, Lowenfels, & Pasker-De Jong, 1993),不過這些發現跟男性女乳症的風險混淆了(Giordano, 2005)。除健康狀況之外,順性別男性用於治療前列腺癌的高劑量雌激素療法也被發現與乳腺癌風險的增加有關(Thellenberg et al., 2003; Karlsson et al., 2006)

關於女性傾向跨性別者的乳腺癌風險的研究項目

本文有一份附件列出了現有與女性化激素療法引起的乳腺癌風險有關的研究項目,其可於此處查閲。截至目前,已有三個大型羣體、數個較小羣體研究評估了女性化激素治療引起的乳腺癌風險。

阿姆斯特丹自由大學醫學中心(VUMC)的研究

附件中大部分研究項目由荷蘭的阿姆斯特丹自由大學醫學中心(VUMC)進行(Asscheman, Gooren, & Eklund, 1989; van Kesteren et al., 1997; Mueller & Gooren, 2008; Asscheman et al., 2011; Gooren et al., 2013; de Blok et al., 2018; de Blok et al., 2019)。該診所為荷蘭 95% 的跨性別者提供治療(de Blok et al., 2019)。VUMC 的研究項目均大致基於同一批持續變化(evolving)的跨性別女性羣體。數十年間,VUMC 僅報告了很低的乳腺癌發生率,並不比順性別男性的高出許多——經過平均約 20 年的激素治療,2300 名跨性別女性當中僅報告兩起乳腺癌病例(Gooren et al., 2013)。該診所通常採用的處方,是雌激素合併高劑量醋酸環丙孕酮。
這些項目並未對乳腺癌做系統性篩查,對乳腺癌的診斷大都依賴於病人的報告,因此存在病例漏檢的可能性(Feldman et al., 2016)

2019 年,VUMC 又進行了一項跟蹤性研究,不過這次使用了新的方式獲得乳腺癌診斷史(de Blok et al., 2019)。研究者不再單純詢問病人是否被診斷出乳腺癌,而是通過遍及荷蘭的一套電子病歷信息系統:“荷蘭國家病理學/解剖學自動化檔案館(PALGA)”來獲取乳腺癌診斷史。這樣一來,其報告的乳腺癌病例數從 2 例躍升至 15 例。這使得接受激素治療平均達 15 年左右之後的乳腺癌相對風險率,相較先前期望的幾乎翻了 50 倍之多,而絕對發生率約達 0.6%。這項發現推翻了該診所早前所有研究的結論,也表明在早前研究中,VUMC 治療的羣體的乳腺癌真實病例數被大幅低估。可以認為,諸如肺癌風險、以及在本項研究驟增近 50 倍的乳腺癌病例等健康風險的大幅增長,應是符合邏輯的。故此,研究者以嚴謹行文,對這種增長做了詳細描述(de Blok et al., 2019; Reactions Weekly, 2019)
在此之前,女性化激素療法所引起的乳腺癌風險之增長被認為很低,而今新的數據證明了其不一定屬實(de Blok, Dreijerink, & Heijer, 2019)

美國退伍軍人衞生管理局(VHA)的研究

另一組大型羣體來自美國退伍軍人衞生管理局(VHA)(Brown & Jones, 2015; Brown, 2015)。2015 年,一篇採用該羣體數據的研究論文被髮表。其中,研究者報告了在平均跟蹤大約 10 年之後,出生指派性別為男的約 3500 人當中有 3 例乳腺癌病例。也就是説,平均治療時長達 10 年時,發病率約為 0.09%。儘管其在平均跟蹤時長上比 VUMC 的羣體更短,但在總時長上與後者相近。VHA 的研究者還報告,乳腺癌發生率較先前預計的增長 33 倍。有些奇怪的是,其和 VUMC 於 2018 年報告的 46 倍並不一致(後者基於 2,300 人平均跟蹤 15 年後有 15 人被確診的事實)。VHA 總結認為,無論如何,女性傾向跨性別者的乳腺癌風險高於順性別男性、也高於 VUMC 早前所報告的,但仍低於順性別婦女。

VHA 的論文並未披露在激素療法當中使用的藥物及其劑量,不過考慮到其囊括了接受 VHA 系統的服務提供者治療的所有患者,其處方應當千差萬別。既然是在美國,通常的處方應該是雌激素合併螺內酯,無孕激素。

VHA 的研究項目有一些問題需要探討:

  • 同 VUMC 早前的研究項目一樣,VHA 的研究也未對乳腺癌做系統性篩查,其診斷乳腺癌基本依賴病人的報告,從而存在病例漏檢的可能性(Feldman et al., 2016)
  • 該項目未將激素療法納入考慮 (Feldman et al., 2016)。VHA 患者羣體中只有少部分人正接受激素治療,研究者並不知道多少人在接受治療、多少人沒有。此外,該羣體中甚至有不明比例的人不是真的跨性別者——研究者將 VHA 系統當中有“跨性別相關”診斷史(如“異裝癖”,或變裝者)的所有順性別男性囊括其中。這些個體的絕大部分應當皆未接受激素治療。
  • 對明確正接受激素治療的人羣的跟蹤時長過短。在 VHA 患者羣體當中,約有 1400 名明確正接受激素治療的女性傾向跨性別者;對其的平均跟蹤時長僅有 5.5 年,而其中近半數個體的跟蹤時長尚未達 3 年。
  • VHA 的論文內容很不明晰,令人費解。例如,VHA 對跨性別者的稱謂有誤,按其出生時的性別“男性”或“女性”指代之並行文,如此很難分辨出誰是誰、誰在接受激素治療與否。不僅如此,連一些針對該研究項目的評述文章也在討論“男性”與“女性”的風險時錯誤地倒置身份(例如 Dente et al., 2019)
  • 還有一點令人費解:VHA 研究者在相同時間點(2015 年)前後發表的兩篇論文裏,報告了包含出生指派性別為男、女的 5100 人的羣體(兩篇所指為同一組)出現 33 例乳腺癌病例(Brown & Jones, 2014; Brown & Jones, 2016)。作為對比,他們在 2015 年的論文中僅報告了 10 例(其中 3 例出生指派性別為男、7 例則為女)。造成這種差異的原因尚不清楚(Braun et al., 2017)

出於上述問題的緣故,VHA 的項目的參考價值有限,可能要謹慎對待。VUMC 在 2019 年發表的資料遠比其更具有價值。

凱撒醫療集團在加州和喬治亞州的研究

第三組、也是最後一組羣體研究,來自凱撒醫療集團(Kaiser Permanente)在美國北加利福尼亞州、南加州和喬治亞州的分站(Silverberg et al., 2017)。他們報告了來自上述三處分站、總計約 2800 名女性傾向跨性別者組成的羣體的不同癌症發生率。對該羣體的平均跟蹤時長只有 4 年。研究者並未報告任何發生不足 5 例的癌症病例;而女性傾向跨性別者的乳腺癌既未出現在相關結果圖表,亦未於討論環節當中被提及。故此,該羣體的乳腺癌病例應該少於 5 例。然而,由研究者 T’Sjoen 及其同行組成的跨性別醫學研究領域權威團隊,在其文章中援引凱撒集團的論文時指出,凱撒集團的團隊發現了女性傾向跨性別者的乳腺癌風險較順性別男性更高、但較順性別女性更低(T’Sjoen et al., 2019)。如果此言屬實(目前來看有很大概率),那麼其應該是從和凱撒集團的研究者的私下交流當中取得的。
凱撒集團的研究存在一定侷限性,包括缺乏對乳腺癌風險的準確估計數字,以及跟蹤時長非常短,等等。此外,癌症診斷史僅取自凱撒集團自有的信息系統;對於其癌症篩查的系統性如何,也尚不清楚。

凱撒集團的研究者計劃進行的研究項目,將通過將其拓展至其它分站來擴大患者羣體規模(Silverberg et al., 2017)。這種改進令人振奮,其有望提供更多和跨性別者健康風險有關的數據,包括乳腺癌風險。

其它較小的羣體研究

其餘的針對女性傾向跨性別者之乳腺癌風險的科研項目規模均較小。其中一項是由比利時根特大學進行的,其跟蹤了 50 名已進行性別重置手術的跨性別女性,平均跟蹤時長 11.4 年;其未報告任何乳腺癌病例(Wierckx et al., 2012)。另一項是由德國埃朗根大學醫院進行的,其跟蹤了 60 名跨性別女性;其同樣未報告任何病例,不過她們的治療時長僅有兩年(Dittrich et al., 2005)。最後一項是由 Harry Benjamin 醫生進行的;他在 1960 年代發表的文章裏透露,其已為大約 150 名跨性別女性提供“中等至較大劑量的雌激素”治療,期限從 3 個月到 12 年不等;其未曾接觸到乳腺癌病例(Benjamin, 1964; Benjamin, 1966; Gooren et al., 2013)
上述羣體規模太小,難以對女性傾向跨性別者之乳腺癌風險進行有意義的量化。

關於既有研究項目的討論

由跟蹤時長過短導致的對乳腺癌風險之量化的不確定性

由於跟蹤時長不足,關於女性傾向跨性別者之乳腺癌風險的現有研究項目的結論並不充分(de Blok, Dreijerink, & Heijer, 2019; Mueller & Gooren, 2008; Gooren, 2011; Gooren et al., 2013; Brown & Jones, 2015)。例如,在 VUMC 和 VHA 的研究裏,平均跟蹤時長僅有 10–20 年。對於順性別婦女,其乳腺癌風險在停經前的數十年裏呈指數級增長,但到了老年,增幅會非常低。順性別婦女通常被確診乳腺癌的年齡為 60 歲,這 60 年裏包括停經前的 50 年激素暴露量、和絕經後 10 年的激素暴露量;如上文所示,這後 10 年裏接受預防性抗雌激素療法可大幅降低乳腺癌風險,因此其重要性不容忽視。綜上,由於現有關於女性化激素療法與乳腺癌風險的研究的跟蹤時長過短,迄今尚不清楚女性傾向跨性別者真實的、或者説終生的乳腺癌風險到底如何(Mueller & Gooren, 2008)

不過我們已知的是,女性傾向跨性別者在使用激素療法(至少也有一種雌激素、一種形如醋酸環丙孕酮的孕激素)約 15 年後,其乳腺癌風險顯然會大幅增加(de Blok, Dreijerink, & Heijer, 2019)。好在,其風險處於順性別婦女和順性別男性之間,並不會高於順性別婦女。然而與此同時,其風險仍不容忽視,絕對發生率也必然隨着跟蹤時間延長而增大。
關於終生風險,至少從 de Blok 等人 (2019) 的發現及其使用的激素治療處方來看,女性傾向跨性別者的乳腺癌發生率大約就在幾個百分點之多。

和終生激素暴露量相關的乳腺癌風險

女性傾向跨性別者的乳腺癌風險應該低於順性別婦女。這可能至少出於前者的終生激素暴露量更少的緣故(Mueller & Gooren, 2008)。事實上,至少從歷史來看,女性傾向跨性別者開始接受激素治療的平均年齡在 30-40 歲,而該年齡距離順性別女性通常經歷青春期的年齡段已過去數十年(Mueller & Gooren, 2008)。此外,青年人羣可能在特定時間段裏對乳腺癌易感(Biro & Wolff, 2011; Biro & Deardorff, 2013; Biro et al., 2020)。然而,近年來女性傾向跨性別者啓動激素治療的年齡正在降低(Mueller & Gooren, 2008),如今有不少人在十幾到二十來歲便開始服用激素了。相較於早年裏服用激素更晚的個體,這些個體的終生激素暴露量會更大,其乳腺癌風險也會更高(Sutherland et al., 2020)。而且,到了順性別婦女通常的更年期年齡,女性傾向跨性別者很少有意停止激素治療(Mueller & Gooren, 2008);其中許多人終其一生皆會維持治療。這種額外的激素暴露量會使乳腺癌風險進一步升高(Mueller & Gooren, 2008)

和孕激素及其劑量相關的乳腺癌風險

現有的發現表明,順性別婦女合併使用雌、孕激素時的乳腺癌風險,要高於單用雌激素;由此可以類推,女性傾向跨性別者合用雌、孕激素的激素療法也會有更高的乳腺癌風險(de Blok, Dreijerink, & Heijer, 2019)。有這樣一種可能:相較於已被 VUMC 研究過的合用雌激素與醋酸環丙孕酮療法,如排除掉孕激素、僅用雌激素,則乳腺癌風險興許會更低(de Blok, Dreijerink, & Heijer, 2019)。另外需要注意的是,VUMC 所用醋酸環丙孕酮的劑量會使得孕激素暴露量相當之高(Aly, 2019)
乳腺癌風險與孕激素劑量之間的關聯程度,目前尚不明確。類似地,乳腺癌風險是否受雌激素劑量影響,以及高雌激素劑量下是否有更高風險,目前也不明確。但無論如何,有絕經前婦女使用雌、孕激素複方避孕療法會增加乳腺癌風險的事實在先,可以認為,較高的雌/孕激素暴露量會增加一定程度的乳腺癌風險(Kahlenborn et al., 2006; Zhu et al., 2012; Ji et al., 2019)
另外,一項早年的調查性研究顯示,絕經前婦女使用雌激素注射劑之後,乳腺癌風險相較口服雌激素、或未服用激素者增加了 4 倍(Hulka et al., 1982; Coe & Parks, 1989)。需要注意,通常劑量的雌激素注射劑所提供的雌激素暴露量,是遠大於口服雌激素和其它給藥途徑的(Göretzlehner et al., 2002; Aly, 2021)。但是,該研究存在很大的侷限性:包括同類型研究僅此一項,年份非常早,等等。

X 染色體與乳腺癌風險

人類性染色體包括 X 染色體Y 染色體。順性別婦女通常有兩條 X 染色體(即 46,XX 核型);而順性別男性、女性傾向跨性別者通常有一條 X 染色體、一條 Y 染色體(即 46,XY 核型)。其中 46,XX 核型(即第二條 X 染色體的存在)可能會是乳腺癌風險的主要因素。X 染色體的獲得(非整倍性)、以及其異常失活,已被發現和乳腺癌的發生及發展有關(Nakopoulou et al., 2007; Di Oto et al., 2015; Lin et al., 2015; Chaligné et al., 2015; Di Oto et al., 2018)。此外,患有克氏綜合徵(即核型為 46,XXY)的男性的乳腺癌風險,也比 46,XY 核型的正常男性要高。
與此相反的是,患有完全性雄激素不敏感綜合徵(CAIS)的女性,迄今未有乳腺癌病例報告——這還是在她們的乳房發育良好的情況下(Aly, 2020);其核型為 46,XY ,和女性傾向跨性別者相同(Hughes et al., 2012; Tiefenbacher & Daxenbichler, 2008; Hughes, 2009)。不過,克氏綜合徵與 CAIS 患者體內的激素異常現象,在乳腺癌風險上的作用可能有所不同,從而會造成一定迷惑性。尤其需要注意的是,患 CAIS 的女性的雌激素暴露量較少,體內孕酮也極少、以至不存在。無論如何,對於克氏綜合徵與 CAIS 患者的乳腺癌風險之差異,無法完全用激素異常來解釋。

結語

女性的乳腺癌風險顯著高於男性。乳腺癌發生率隨年齡增長呈指數級升高,但在年輕人羣當中非常少見。有強力的證據支持以下觀點,即雌激素與孕激素會增加乳腺癌發病風險;在因性別造成的乳腺癌風險差異當中,其可解釋為主要成因機理。現有數據並未表明,在對乳腺癌風險的作用上,具生物同質性與不具生物同質性的激素有何明顯差別。

早年有關女性化激素療法與乳腺癌風險的研究發現,女性傾向跨性別者的乳腺癌發生率很低。然而,近期的一項研究採用了更好的研究方法,其發現的乳腺癌風險增加了近 50 倍。其患者羣體所用的激素治療處方,僅為雌激素合併醋酸環丙孕酮。目前尚不清楚其它激素處方(如單用雌激素,或雌激素合併非甾體類抗雄製劑)是否會有相似的風險。由於跟蹤時長不足,有關女性傾向跨性別者之乳腺癌風險的現有數據存在侷限性。因此,其終生乳腺癌風險尚未可知。
不過基於現有數據,可以預測未來的情況,即女性傾向跨性別者的終生乳腺癌風險率應有幾個百分點之高。該風險率處於順性別男性和順性別婦女之間。

女性化激素療法引起的乳腺癌風險會受到不同因素的影響。例如,累計用藥時長,開始治療的年齡,孕激素的長期服用,以及(可能的)雌/孕激素劑量,等等。女性傾向跨性別者體內第二條 X 染色體的缺少也許對乳腺癌起到了部分抑制作用。
儘管女性化激素療法引起的乳腺癌風險是有必要引起女性傾向跨性別者關注的(特別是考慮到孕激素在其中的促進作用),但放眼長期,乳腺癌的終生髮生率應該很低,其風險顯然也低於順性別婦女。此外,乳腺癌的發生也需要經過長達多年的激素暴露,其一般也僅出現在老年。而且,乳腺癌容易治療,患者生存時長可達 5-10 年(Cancer.Net)。綜上,基本可以肯定,考慮乳腺癌時不應將女性化激素療法的情況排除在外。

女性化激素療法引起乳腺癌的可能性,倒是凸顯了對年齡與激素暴露量符合標準的女性傾向跨性別者定期進行乳腺癌篩查的重要性(Chowdhry & O’Connell, 2020)。應當建議長期進行激素治療的女性傾向跨性別者,也像順性別婦女的過程一樣接受乳腺癌篩查(Chowdhry & O’Connell, 2020)
