摘要

目前為止,全球已有數家性別診所成功應用了雌二醇的舌下含服途徑——儘管尚鮮有醫學文獻對其加以描述。舌下含服途徑儘管可能存在一些實際不足之處,但在適當劑量下,其有效性與經濟性應該能為臨牀經驗與權威評價所證明。對於部分女性傾向跨性別者,舌下含服雌二醇應當可以代替口服,用於女性化激素療法。

前言

口服雌二醇片劑的最常用途徑是口服,不過也可通過舌下、面頰含服(Kuhl, 2005)。所謂舌下含服是指,將片劑置於舌頭下方,使其溶解並被吸收,進入血液循環。而面頰含服與此類似,其需將片劑置於嘴唇與牙齦之間,片劑在此也會快速溶解並被吸收(Gass et al., 2004; Bartlett & Maarschalk, 2012)

許多女性傾向跨性別者會想知道有關舌下含服雌二醇的詳情;在網上詢問最多的問題有“我應該使用多大劑量的含服雌二醇?”“我應該隔多久含服一次?”“舌下含服比口服要好嗎?”等等。

在早前,筆者曾對有關口服與舌下含服雌二醇之間之比較的文獻作了評述(Sam, 2020a)。儘管不少女性傾向跨性別者已有舌下含服雌二醇的經驗,對於自行用藥者亦然,但筆者未在該評述當中提及舌下/面頰含服。其中部分原因包括,對此類不同於傳統上更常用的途徑的雌二醇用法給予關注的研究特別少;這貌似也是許多臨牀實踐指南未囊括舌下含服雌二醇的原因所在(Aly, 2020a)。而且,當時筆者希望只專注於口服雌二醇。
不過,在該評述發表之後,有好幾篇關於女性傾向跨性別者舌下含服雌二醇的研究論文都發表了。有了這些新資料,筆者覺得有必要針對女性傾向跨性別者常見的問題,來深入剖析此類用法;於是便有了本文。本文對已發表的有關舌下含服雌二醇——尤其是其在跨性別護理方面的應用——的醫學文獻進行概述,以期為上述問題進行釋疑。

需要指出,儘管舌下含服與面頰含服完全是兩回事,但其作用機理與藥理學特性非常相似(Perloff, 1950; Chandrasekhara et al., 2002)。因此,雖然筆者在此主要討論的是舌下含服,但其中許多內容同樣適用於面頰含服途徑。

舌下含服雌二醇的藥理學特性

目前舌下含服雌二醇在臨牀實踐當中的使用,並不及口服或其它形式的雌二醇要普遍;不過,迄今已有一批研究項目對這種形式的藥理學特性做了考察。這些項目的受試者包括已絕經順性別婦女、女性傾向跨性別者及其他患者羣體(Casper & Yen, 1981; Serhal & Craft, 1989; Deutsch, Bhakri & Kubicek, 2015; Cirrincione et al., 2021)。口服雌二醇、戊酸雌二醇片劑均可用於舌下含服(Serhal, 1990)

口服雌二醇之後,片劑成分會被肝臟大量代謝並失活,成為雌激素結合物(Kuhl, 2005)。這些代謝物反過來會逐漸轉換成雌二醇,使得雌二醇半衰期延長(約為 13-20 小時)(Stanczyk, Archer & Bhavnani, 2013)。舌下含服雌二醇則與此相反:其不會大量經過肝臟,也不會因肝臟代謝而大幅失活;其很快就被吸收並直接進入血液循環。因此,舌下含服雌二醇的生物利用度要高於口服;這意味着只用較低劑量,便可達到相近的雌二醇累積水平(Kuhl, 2005)(見下圖)。這是舌下含服相較於口服的一項優勢,其對劑量需求更小、藥費也更少。

由於在舌下含服過程中可能會不慎吞服部分雌二醇,故舌下含服途徑很可能囊括了含服與口服給藥的情況(Lobo, 1987; Kuhl, 2005)。一項正在進行的有關女性傾向跨性別者的研究在其初期報告裏寫道,單次舌下含服 1 mg 雌二醇之後的 1-2 小時內,雌二醇水平皆有所上升,最高達 179 pg/mL 以上;與此相反,口服 1 mg 雌二醇僅可達到 36 pg/mL 的水平峯值(Doll et al., 2020);此後雌二醇水平快速下降。另一些針對已絕經婦女的研究,同樣觀測到了類似的大幅上升的濃度峯值(Burnier et al., 1981; Price et al., 1997; Wren et al., 2003)。雌二醇水平上升時,增幅被發現可達口服的十倍左右,隨後快速下降,而消除半衰期僅有幾個小時(Kuhl, 2005)。在這方面,舌下含服雌二醇與靜脈注射途徑較為相似,後者在注射後也引起雌二醇水平的快速上升、消除半衰期則非常短(Kuhnz, Gansau & Mahler, 1993)。同樣類似的是經鼻腔給藥途徑(Devissaguet et al., 1999)
由於尖峯較大、作用時間短,舌下含服雌二醇在濃度上的表現並不如其它劑型那樣穩定。因此,這是和其它劑型的明顯不同之處——像口服雌二醇在一天裏就很少有激素水平上的波動。

已有研究報告了對舌下含服雌二醇之生物利用度的估計範圍。其中一項針對已絕經婦女的小型隨機性研究發現,在同等劑量下,舌下含服雌二醇引起的雌二醇累積水平較口服高出約 2.5 倍(Price et al., 1997)。另一些研究則報告,舌下含服雌二醇的生物利用度估計值相對口服可達 5 倍之多(Pines et al., 1999)。一項以絨猴進行試驗的研究發現,舌下含服雌二醇的絕對生物利用度為 10%;此數字約為傳統上對口服雌二醇的生物利用度的估計值(5%)的兩倍(不過範圍可達 0.1-12%)(Kuhnz, Blode & Zimmermann, 1993)。因此,在雌二醇累積水平上,舌下含服途徑應相當於口服的 2-5 倍。
基於上述發現,可以得出用於女性化激素療法的舌下含服雌二醇劑量(見下表)。需要指出,因缺乏相關研究,舌下含服雌二醇的相對效力尚未被準確描述。

舌下含服雌二醇與女性化效果

由於舌下/面頰含服雌二醇的半衰期很短,一些與在女性化激素療法使用之有關的問題浮出了水面。在網絡論壇上,有一個最常見的問題是:哪種性別肯定激素治療的處方是最“有利於”雌激素產生女性化效果的——其中包括(而不限於)乳房發育和脂肪分佈的效果。

目前尚無任何數據描述舌下含服雌二醇引起的女性化程度;相比之下,口服及透皮雌二醇已有所描述(Sam, 2020a)。不過,如上文所述,在女性傾向跨性別者及性腺發育不良的順性別女孩的乳房發育及其它女性化效果上,口服與非口服形式的雌二醇並未被發現有任何差別(Rosenfield et al., 2005; Shah et al., 2014; Klaver et al., 2018; de Blok et al., 2021)。因此,對於舌下含服的雌二醇,應該也不會在效力上有別於之。
迄今已有多家性別診所使用舌下含服雌二醇;這一事實令人振奮,其表明舌下含服雌二醇可有效促進女性化(Deutsch, Bhakri & Kubicek, 2015; Lim et al., 2019; Cirrincione et al., 2021)。而且,鑑於迄今尚無任何對比並客觀評價舌下含服雌二醇之女性化效力的研究,目前根本説不準口服與舌下含服雌二醇在女性化效果上到底有任何差別。但願將來會有更多研究聚焦於此。

舌下含服雌二醇對睾酮的壓制效力

因為舌下含服雌二醇的半衰期很短,還催生了另一個問題:在對睾酮及其它雄激素的壓制能力上,舌下含服途徑和其它傳統途徑的對比如何。

最早在 1940 年代,雌激素首次被描述為具有抗促性腺激素作用的抗雄激素製劑;其以口服合成雌激素(即己烯雌酚)的形式,用於治療男性的前列腺癌(Huggins & Hodges, 1941)。而口服炔雌醇、一些長效雌二醇酯(如聚磷酸雌二醇)以及雌二醇透皮貼片,也曾有相關研究;其抗雄效力已確認無疑(Stege et al., 1996; Kohli, 2006; Sciarra et al., 2015)。由於有關女性傾向跨性別者使用雌激素壓制睾酮的數據稀缺,上述數據對女性化激素療法會很有啓發。
而另一方面,舌下含服雌二醇從未被用於治療前列腺癌,因此尚無任何數據表明其抗雄效力如何。

近期一些研究發現,在許多女性傾向跨性別者身上,僅由非舌下含服形式達到生理水平(即 100-200 pg/mL)的雌二醇,足以快速且接近完全地將睾酮水平壓制到女性範圍(Leinung, Feustel & Joseph, 2018; Pappas et al., 2020)。另外,有一項正於英國進行的多中心隨機對照試驗研究,對約 900 名男性使用前列腺癌透皮激素療法(PATCH);其提供的新數據顯示,通過透皮貼片維持雌二醇水平中位數在 215-250 pg/mL 之間,可將睾酮水平壓制到閹割範圍(<50 ng/dL),降幅約 95%,效力與促性腺激素釋放激素(GnRH)類似物相當(Langley et al., 2021)
然而,考慮到舌下含服雌二醇在藥代動力學上的顯著差異,可以認為,在雌二醇累積水平相當的劑量下,舌下含服雌二醇對睾酮的壓制作用可能低於其它途徑的平均水平。換句話説,在睾酮壓制作用上,舌下含服之後波動很大、且作用時間更短的雌二醇水平,可能不及更穩定的雌二醇水平那樣有效。

對此有一些藥理學研究可略為佐證。其中一項證據是:一些關於婦女使用舌下含服與靜脈注射雌二醇的研究報告,儘管雌二醇水平在數個小時內充分升高,也能較快產生對促性腺激素(即促卵泡激素與促黃體激素)的壓制作用,但該作用相對有限(Tsai & Yen, 1971; Burnier et al., 1981; Casper & Yen, 1981; Hoon et al., 1993)。這些研究質量較差,而且由於未測量睾酮壓制作用幾何、受試者也僅有順性別婦女,其證據也不夠明瞭。另一個問題在於,這些研究均僅安排單次用藥,其結果不大能代表多次用藥的情況。有這樣一種可能:重複以生理劑量使用舌下含服雌二醇,便可使促性腺激素壓制效果最大化。
然而,這些研究可能表明:對於女性傾向跨性別者,如果舌下含服雌二醇的劑量不夠大、且不使用其它抗雄製劑,則不足以最大程度地壓制性腺功能;而大劑量又會帶來更高的健康風險。

還有一種可能:在與有關前列腺癌的研究所採用的劑量(如 2-4 片 100 μg/24小時的透皮貼片)相當的情況下,舌下含服的單雌二醇療法可能不及前者有效。因此,對於單雌激素療法的處方,使用雌二醇透皮貼片、凝膠,或者非腸道途徑的雌二醇酯(如戊酸雌二醇注射劑),可能更為可靠。
另一方面,已有研究者對女性傾向跨性別者使用舌下含服雌二醇(合併或不合並低劑量醋酸甲羥孕酮,即 MPA——一種合成孕酮製劑)進行了研究(Jain, Kwan & Forcier, 2019)。研究顯示,當女性傾向跨性別者同時使用舌下含服雌二醇與 MPA 時,睾酮水平均至少有相當程度的下降,並降至女性範圍(<50 ng/dL);這表明如果合併其它抗雄製劑(如 10 mg/天的醋酸環丙孕酮)使用,舌下含服雌二醇很可能足以有效地壓制睾酮(Aly, 2019)

對舌下含服引起的雌二醇水平的監測

由於舌下含服雌二醇引起的雌二醇水平的大幅波動,也進一步產生了是否通過驗血來監視雌二醇水平的考慮。截至目前,所有《指南》均未對用藥後何時進行驗血作出建議(Deutsch, 2016; Cheung et al., 2019; T’Sjoen et al., 2020)。這可能和實際需求有一定關係;或者是因為目前尚未有來自任何隨機性對照試驗的、可供指引女性化激素療法所用劑量的可靠數據。而且,也由於舌下含服後的雌二醇水平波動很大,有必要知道用藥後何時進行採血,以更準確地解讀來自實驗室的結果。

例如,如果剛用藥便去測定激素水平,則很可能會引起誤解,因為此時測得的雌二醇水平會特別高,從而促使用藥者為控制雌二醇於合理水平而減小其劑量。事實上,這是由舌下含服雌二醇的藥理學特性引起的誤解:因為這個採樣時間點幾乎正值雌二醇水平最高之時。如此雌二醇水平無法代表其平均暴露量——後者才能更準確地衡量總雌激素效力。
類似地,如果在時間軸的另一端——即即將用藥時——進行採血,也可能會引起“雌激素水平過低”的誤解,從而過量服用雌激素。此類誤解在不熟悉舌下含服雌二醇的藥理學特性的人羣當中更為常見:因為雌二醇水平僅在用藥後最初幾個小時內維持高值,此後快速下降。

有關舌下含服雌二醇的研究也存在這種疏忽的跡象。例如,在上文提到的對性別肯定激素療法使用舌下含服雌二醇進行的分析結果當中,並未説明其用藥頻率與用藥之後何時進行測定(Jain, Kwan & Forcier, 2019)。儘管其數據非常寶貴、並且對在這方面持續進行的研究具備參考價值,但是並沒有解答用藥後何時測定雌二醇水平這個關鍵問題。當下很難再對該研究所記錄的雌二醇水平作任何評述;而且,無法利用其數據來得出口服與舌下含服雌二醇在何種劑量下(效力)相當。

為了解決因舌下含服後雌二醇水平快速波動而導致的上述問題,或許只需選擇在雌二醇水平大概最接近於平均值的時間點進行測定。相關研究發現,對於舌下含服雌二醇,這個時間點一般在服藥後四小時左右——不過個體之間差異可能會很大(Kuhl, 2005)。而且,通過這種方式,可以獲得最能代表總雌激素暴露量的“大致印象”,也能夠幫助使用舌下含服雌二醇的人避免對實驗室(測定)數據產生誤解。

關於每日分多次含服的説明

為補償舌下含服雌二醇的較短半衰期,可以每日以小劑量分多次服用,以維持稍為更穩定的激素水平(Ahokas, Kaukoranta & Aito, 1999)

在一項研究當中,在高劑量雌激素療法下,每日舌下含服 3-4 次 2 mg 雌二醇(總劑量 6-8 mg/天)的絕經前婦女的激素水平,相比每日服一次者顯著穩定得多(Serhal & Craft, 1989)。另一項研究安排每日服用 3-8 次,也復現了上述結果(Ahokas et al., 2001)
與此相反,在有關低劑量面頰含服雌二醇的研究中,有三分之一都使用每日含服 1-2 次 0.25 mg 雌二醇;在 12 小時觀察時段內,其發現無論含服 1 次還是 2 次,其雌二醇濃度“穩態”都沒有明顯差別(Wren et al., 2003)
上述發現表明,舌下/面頰含服雌二醇可能要每日分至少三次服用,以使雌二醇水平更為穩定。

基於上述理由,如果女性傾向跨性別者要使用舌下含服雌二醇,一日內分多次服用的方式會更適合;或許最好每日至少分 3-4 次。例如,比起一次性服用 2 mg,如在 24 小時內分四次服用 0.5 mg,應該會更佳(在可行範圍內儘量讓間隔均等)。雖然每日分多次含服可能會帶來一定不便,但至少雌二醇水平會因此更加均衡、睾酮壓制作用也可能更好。
每日分多次服用的方式應視為舌下含服雌二醇的最佳用法。

安全性與耐受性

遺憾的是,在與舌下/面頰含服雌二醇的藥代動力學有關的安全性與耐受性方面,現有的醫學文獻尚顯不足。不同於口服及透皮雌二醇(已在圍絕經期婦女身上進行嚴謹研究),當下尚無任何有關舌下含服雌二醇的長期安全性數據(Rovinski et al., 2018; 乳腺癌激素相關因素合作研究小組, CGHFBC, 2019)

雌激素的不良健康影響

由於對肝臟的一系列影響,雌激素在暴露量足夠的條件下,會引起血栓風險的上升(Kuhl, 2005; Aly, 2020b)。此外,雌激素在特定條件下還會引起其它心血管併發症(Anderson et al., 2004; Mikkola et al., 2005)。這些是跟性別肯定激素療法相關的最大的健康顧慮——儘管其絕對風險在短期內很低。

僅有少量研究評估了舌下含服雌二醇對肝臟的影響(Pines et al., 1999; Lim et al., 2019)。數據發現其在脂質與膽固醇上的作用與其它形式的雌激素相似。有一項證據表明舌下含服雌二醇對肝臟的作用要強於透皮雌二醇等其它形式;那就是其引起的雌酮與硫酸雌酮含量顯著更高——也即,其在肝臟內引起的雌激素暴露量更大(Burnier et al., 1981; Cirrincione et al., 2021)。非口服形式的雌二醇在高劑量下可引起高凝狀態,從而在肝臟引起強烈的雌激素活動。

有相當一部分的研究對雌激素的短期與長期健康影響有所關注,不過無一研討過舌下/面頰含服的情況(Oliver-Williams et al., 2019; Mishra et al., 2021)。由於口服雌二醇的風險比非口服形式更高、且舌下含服只是部分(而非完全)避免了肝臟首過效應的代謝作用,可以認為,舌下含服雌二醇的風險應不高於口服、但也不低於其它非口服途徑。
一項在美國進行的回顧性羣體研究發現,修正干擾因子後,在血栓栓塞發生率上,正服用平均 4 mg/天的口服雌二醇的女性傾向跨性別者約為對照組(未服用激素的順性別婦女)的兩倍;相對對照組的風險比率為 2.0(95% 可信區間:1.4-2.8)(Getahun et al., 2018)。此類風險之增幅已經遠比早年女性傾向跨性別者所用處方(含高劑量合成雌激素)的要小;但同時需要注意,即便是此等增幅也會大幅增加發病率和死亡率(Morimont, Dogné & Douxfils, 2020)
在此建議限制舌下/面頰含服雌二醇的劑量(如小於 6 mg/天),這樣它們不至於產生過多有害作用,並可令性別肯定激素治療的益處與風險處於平衡。

用藥者的不配合行為

對於女性傾向跨性別者,舌下含服有一項很現實的阻礙:即每日需含服三次、四次乃至更多,會帶來相當程度的不便。一些觀察性研究發現,一般而言,為患者開出的藥量與每日劑量,和患者的不配合行為及漏服次數呈正關聯(Jin et al., 2008; Toh et al., 2014)。這些發現尤其跟女性傾向跨性別者有關,因為一般情況下,我們需要在數十年間一直服用激素。如果一段時間裏僅漏服一次,倒無傷大雅;但連續多次漏服的話,問題就大了。

與舌下含服相反的是,口服雌二醇及其透皮凝膠的半衰期長至足以僅需每日用藥一次(Wiegratz et al., 2001; Potts & Lobo, 2005)。至於透皮貼片和非腸道給藥(如注射劑——譯者注)形式的雌二醇,其補充間隔可長達數日、以至更長(Thurman et al., 2013; Wisner et al., 2015)。因此,如果女性傾向跨性別者準備使用舌下含服而非其它途徑,也應當格外留意,每日都要多次含服是否符合實際、足夠方便;如否,則其它劑型應更適合使用。如要長期用藥,那就更應該考慮這點了。

總結

相比於其它雌激素形式,當下對於舌下/面頰含服雌二醇的研究還很少。歷史上,諸如口服、透皮雌激素等形式,常用於緩解圍絕經期症狀及其它用途;因此,在女性化激素療法當中,這些形式得到了遠甚於舌下含服形式的關注度與特性描述。不過,近期數項研究填補了我們對舌下含服雌二醇的認知空白。如果有跨性別健康臨牀實踐指南未曾討論舌下含服雌二醇之使用的話,其應當考慮在相關信息愈發完善之後修訂其內容。

在藥理學特性上,舌下含服雌二醇與其它劑型不同。其中最主要的差別是:其會引起雌二醇水平的快速上升和下降。其生物利用度約為口服的 2-4 倍,因此其僅需 25-50% 的劑量* 便可提供相同的雌二醇暴露量。這不僅使得其開銷低於口服,而且同等劑量下也可達到更高的雌二醇水平,有助於壓制睾酮,因此具備一定優勢。

目前尚無任何證據顯示或表明,在女性化效果上,舌下含服雌二醇是比其它途徑更好還是更差;不過可以確信的是,在同等雌二醇總暴露量下,舌下含服雌二醇對睾酮的壓制作用可能稍弱。儘管如此,當合並服用其它抗雄製劑時,舌下含服雌二醇還是能夠有效壓制睾酮。
使用舌下含服雌二醇時,如要監測雌二醇水平,需要留意(檢測時機)以確保其結果可被準確解讀。為儘量讓上述潛在問題變得最小,可以每日分多次含服。

迄今尚無任何大型觀察性或隨機性研究對舌下含服雌二醇帶來的健康風險進行測定。因此,其引起心血管問題的風險尚未可知——儘管其一部分引起的肝臟首過效應在這方面而言是安全的。
舌下含服雌二醇的使用(尤其是長期使用)可能不大方便,這時可以考慮換用其它劑型。

總而言之,舌下含服雌二醇應具有許多優良特性,可代替口服雌二醇用於女性化激素治療——儘管其顯然尚需更多研究來得到更準確的描述。
